Grade 9 Identity Wheel Worksheet (with Answer Key)

About This Resource

A 2-page worksheet built around an original 8-slice identity wheel graphic, where students label the parts of identity, map their own wheel, and reflect on which part matters most to them — with a full answer key.

Overview

Students begin by studying an illustrated 8-slice identity wheel and labeling each icon with its correct part of identity (name/background, family/heritage, personality, values, interests, skills, goals, and relationships). Part B has students complete their own identity wheel as a table, writing one true detail about themselves for each part. Part C is a short reflection asking which part feels most important right now, and a closing Quick Check and Extension add a light discussion element.

Student Task

  • Use after the Vocabulary PowerPoint's identity-wheel slides and alongside the Flashcards for extra review. Part A works well completed together as a class; save Parts B and C for independent or partner work.

Classroom Use

  • Identity Wheel Gallery Walk: once students finish Part B, post the wheels around the room and have students do a silent gallery walk, leaving one sticky-note compliment on a classmate's wheel.

Differentiation

Support: Allow students to complete Part B with a partner, and provide a sentence frame ("One part of my ___ is ___.") for each row.
On-Level: Students complete the worksheet independently, moving from labeling to personal reflection.
Challenge: Ask early finishers to interview a partner and complete a second identity wheel table describing that partner instead of themselves.

Answer Key & Assessment

Collect Part A as a quick written check of the 8 identity parts. Score Part C's reflection with the Grade 9 Personal Identity Unit Grading Rubric, since it is open-ended.

Learning Objectives

  • Students will be able to name the 8 parts of personal identity.
  • Students will be able to write a true, specific detail about themselves for each part of identity.
  • Students will be able to explain in writing which part of their identity feels most important to them.

Teacher Tips

Before Part B, share one detail about your own identity wheel as a model — hearing a real, specific teacher example makes the abstract categories much easier for students to fill in about themselves.
